Registration as a Member

  1. Individual Member - An individual member is anyone joining cfb Medical Centre in his/her own right and not through a company.
Subscriptions for individual members are payable for six months in advance, and subsequently payable quarterly, at a rate determined by the Board of Directors. Subscriptions, once paid, are not refundable, but can be transferred. A one-off capitation fee is also payable at the time of joining.
  2. Corporate Members - A corporate member is one who has membership of cfb Medical Centre through the company he/she works for, or is a dependent of such an employee.

Additions & Deletions - Individual Members

Additions of new members must be made in writing, and failing such notification the individual will not be treated. Individual members wishing to include family members during the course of their paid-up quarter are expected to pay the balance of the quarter, and also pay in advance for the following quarter.

Deletion of members must also be notified in writing, and failure to do so will result in subscription charges continuing until notification is received.

Additions & Deletions - Corporate Members

Additions and deletions of members must be notified in writing as they occur failing which charges will be made up to the date of advice.

Expiry of Clinic Membership - Individual & Corporate Members

Clinic membership will be suspended on expiry of paid up subscription and will be terminated if payment is not made within one quarter. If terminated, it will be required that the capitation fee be paid if membership is reinstated at a future date. People whose clinic membership is suspended will be referred to Accounts to clear payments before seeing a doctor.
