Expiry of Hospital Membership

Hospital membership expires on the last day of the paid up period. Please note there is no "grace period" applicable.

Newborn Infants

Newborn infants may be covered under the mother's membership from birth up to two weeks of age - for conditions see "Services covered by Clinic Membership", and "Services covered by Hospital Membership".

Membership Cards

Every member of the clinic and/or hospital will be given a membership card. This card, plus additional identification such as a Registration Card, driving license or passport must be presented at all times before treatment can be received.

Payment of Fees in Advance

Payment of fees in advance is welcome.

If a situation arises whereby a member is deceased, then the fees are transferred to another family member.

The above rules apply to both clinic and hospital.

Member's Responsibilities - Members are required to:

  1. Make an appointment wherever possible, by telephone or in person, to avoid a long wait to see your doctor
  2. Be punctual for an appointment
  3. Carry their current Membership card and ID at all times
  4. Abide by the clinic hours except in an emergency
  5. Make a review appointment before leaving the clinic
  6. Make sure their account is paid up to date
  7. Be advised not to use your cell phone within the clinic or hospital as this may disrupt other patients and equipment. Cell phones are often high theft items and their use also interferes with electronics. We therefore suggest leaving yours at home if you are admitted
  8. Pay outstanding hospital charges before discharge
  9. Abide by cfb's clean-air, no smoking policy
  10. Respect hospital visiting hours
  11. Children under 12 years of age must be accompanied by an adult at all times
  12. Children under 12 years of age are not allowed to visit patients in the hospital unless prior permission is obtained

NB: Only emergencies will be attended to after normal outpatient hours.

Right of admission is reserved.
In disputed situations the decision of the Board of Directors will be final.
All charges and rules are subject to change.
